CONCIERGE SERVICES

These services were a result of something I saw a need for from my personal and professional experiences. At this time we only offer Senior Check-in services (in person or via phone). Although we want to be there for our loved ones(and we truly do), we can't be for many reasons (i.e. busy work schedules, family obligations, personal health issues, distance). The Senior Check-ins are awesome because I can fill that void for you and ease the burden of knowing if you're loved one is doing okay. It can be long term or short term. It allows for some socialization and gives them something to look forward to. With over a decade of home care nursing experience and visiting thousands of patients at this point I know what to look for and how to ask questions to get the answers I need. There's a way I make my client's comfortable that they always want to open up to me. Families are always astonished after my visits the information their loved ones give me and never give to them. I love when they answer my question by saying "Nurse I can't lie to you...." then proceed to confess whatever is on their heart. I appreciate that honesty and I don't take their trust for granted.

- Senior Check-in (in-person visits): I visit and "check-in" on your loved one once a week. A brief evaluation of the home, home attendant (if they have one), and your family member is performed. I sit and talk with your loved one to discuss any changes. This also allows for socialization and helps them forget about the feelings of loneliness they may be dealing with. We can arrange for me to video conference you (if you're available) during my visit so you can see your loved one for yourself. This is great especially if you live far away. You will receive an email update after each visit. These check-in visits can take place in the home, at an independent living facility, an assisted living facility, a rehabilitation center, or a nursing home. The actual visit is up to 30-45 minutes in duration. I do not visit hospitals at this time. I will be your eyes and ears with my nursing eyes and ears. Please note that locations other than the home environment will likely not allow me to get detailed health information on your loved one due to the facilities privacy policies unless you arrange with them otherwise in advance.
- Senior Check-in (via telephone): Your loved one receives a brief daily phone call from me to check on their well being. Calls are around 1-5 minutes in length. We talk about whatever they want to talk about. That can be from the weather to politics to memories from when they were a child. During the call I also ask questions that will alert me to any changes in their mental or physical state. My nursing ears become your ears. The call gives them something to look forward to and take their mind off of feeling lonely (even if for a few minutes). An email is sent to you afterwards with an update of your loved one's health condition that day.
RATES
Note that I am based in Queens, New York so in-person Senior Check-in services are available to those that live in Queens, Brooklyn, Nassau County and Western Suffolk County (up to Islip). Visits to other areas of NYC or Eastern Suffolk County (past Islip) can be arranged but will have an additional fee (contact me to discuss).

- One-time only visit or one visit per month...... $150 per visit (15% off when monthly visits are paid in advance for 3 months or more)
- 2 to 4 visits per month (one visit per week)....... $119 per visit (15% off when paid in advance for 3 months or more)

- One Call (7 days/week)...$45/month
- Two Calls (Monday to Friday)...$50/month
- Two Calls (7 days/week)...$80/month
