CONSULTATION SERVICES
The following services offered can be done in-person or virtually. I am based in Queens, New York so in-person services can be provided to clients in the areas of Queens, Brooklyn, Nassau County, and Western Suffolk County. There is an additional travel fee for in-person services in Staten Island, the Bronx, Manhattan and Eastern Suffolk County(past Stonybrook).

STRATEGY SESSION

THE HOME ATTENDANT HAS STARTED(or is starting soon)......NOW WHAT?! You've never had a home attendant before. What do they do? How can they assist? What happens now that they are in the home? We arrange to have a one hour strategy session via phone or video call.

It's a major adjustment when you have a home attendant in the home. 9 times out of 10 it's someone you've never met and this is something you're totally new to. Take a deep breath and know I am here to assist you during this time. My nursing career spans over a decade in the homecare specialty alone so I've learned a thing or two over the years. I plan to give you some tips and tricks you need to know entering this new chapter in your life. After getting some background on you and your loved one, I will discuss general details as well as topics that will be relevant to your specific situation.

HOME SAFETY ASSESSMENTS

Prevention is key in helping seniors stay in their homes longer also known as "aging in place". The assessment is conducted in and outside the home using a 120 item checklist. Each room is discussed in detail as we walk-through the home together. The visit takes about 60-90 minutes depending on the home size. The client or family is emailed the final recommendations/comments 2-3 business days following the home assessment to use as a reference in improving the safety of the home. As I tell my patients all the time....It only takes one fall to change your entire life and home dynamics. Let's work together to prevent that the best we can today.
ADVANCE CARE PLANNING SESSION

Preparation is key in advance care planning. We need to be as prepared as possible for unplanned incidents or if we get that final call to meet our maker(aka death). No one is promised tomorrow so lets do what we can today. I offer advance care planning in two ways--a consultation call and/or a sit-down fill in session.
CONSULTATION CALL
Schedule a consultation phone call (45-60 minutes) where I explain in detail what advance care planning is and we discuss what you have (or don't have) in place. After the call you will receive an email link to the phone call recording. You will also receive an email with notes of what was discussed and recommended for you to get your affairs in order. This will allow you to get started and have something to reference back to if you forget later what was discussed.
SIT DOWN/FILL-IN
It's totally free to complete your advance directives yourself (visit www.aarp.org ). Many people will visit the suggested site, half of those people will print out the documents, and less than half of those people will actually finish the task of filling out the forms. I want people to complete their documents. I need people to complete their documents.
With the "Sit Down" session, I sit with you (in person or virtually) to pre-fill those documents (for your state). We will also fill out the "Get It Done Now!" Advance Care Planning Workbook that I created (see section below) following my own family crisis I had with my mom's heart attack in 2015. At the end of the visit you will be left documents in the home (in-person visit) placed in a 3-ringer binder or binder will be mailed to you if visit was done virtually.
"GET IT DONE NOW!" ADVANCE CARE PLANNING WORKBOOK
I created this 15 page detailed workbook after my mom had a major life changing heart attack in 2015. I realized I knew nothing regarding my mother's affairs since she was more than able to handle them herself. She was in a medically induced coma for over a week and in the hospital for even longer. All this is going on and bills still had to be paid. My brother and I didn't even know where to start. After stabilizing my mom, I vowed no one should have to experience what I went through so I started the "Get It Done Now!" Movement to bring awareness to advanced care planning. The workbook was created to include items we don't think of in an emergency, but we likely will need knowledge of including sections for medication, doctor's information, last wishes, insurance, work information, financial information, etc...
