- Intake review of medications, health history, and surgical history
- Up to 90-minute visit
- Comprehensive 240-point home safety assessment covering bedrooms, bathrooms, kitchen, living/dining areas, hallways, and outdoor spaces
- Summary and recommendations report, emailed within 24 hours
- Follow-up call to discuss recommendations with family, if requested
