MEMBERSHIP PLAN

Dear 2 My Heart offers monthly membership concierge nursing services to help make life a little easier for our aging seniors and their families.  Although families want to be there for their loved ones (and we truly do), they may not be able to for various reasons (i.e. busy work schedules, family obligations, personal health issues, distance).
​The Aging In Place Wellness Visits are beneficial because I can fill that void for you and ease the burden of not knowing if you're loved one is doing okay. The visits also allow for socialization and human interaction. 
Sometimes all you need is someone to lay eyes on your loved one and reassure you they are okay or inform you of any issues that may be happening. Let my nursing eyes and ears be your eyes and ears. 
AGING IN PLACE WELLNESS VISITS
4 hours per month
1 hour weekly in-home visits
Monthly Nurse Assessment
Social/Environmental Assessment
Monthly Medication Review and Education
Research of diagnosis or health information
Review medical/test reports/medical visit summaries
Vital Signs taken every visit
Companionship
Care Coordination
Communication with medical team as needed
Video calls with family
​Weekly visit summaries emailed to family

$600 for month to month 
$1500 for 3 months 

  • No insurance accepted. Payments by debit/credit card only.
  • The rates above are for a membership plan for one person
  • Wellness visits at this time are only to residential locations. Due to Covid-19, facilities in New York are not allowing visitors.

Aging In Place Wellness Visits are available to seniors that live in lower New York state in Queens, Brooklyn, Nassau County and Western Suffolk County (up to Islip). Visits to other areas of New York City, Westchester County, or Eastern Suffolk County (past Islip) can be arranged, but will incur an additional travel fee (contact me to discuss).
